If you know someone who needs any of our services, you can make a referral to us on their behalf. Services that we provide that you can make a referral for:

• Fall prevention
• Medication reconciliation
• Assistance getting medications
• Housing assistance
• Assistance with elder/child abuse or neglect
• Community resource navigation
• Narcan (Naloxone) kits
• Medications for Opioid Use Disorder Program

Contact our Community Paramedic Staff Members by phone at 910-893-0721. Our staff are routinely in the field assisting patients, so please assure you leave a message with your name and a phone number so they can return a phone call to you to discuss the referral.
