If you know someone who needs any of our services, you can make a referral to us on their behalf. Services that we provide that you can make a referral for:
- Fall prevention
- Medication reconciliation
- Assistance getting medications
- Housing assistance
- Assistance with elder/child abuse or neglect
- Community resource navigation
- Narcan (Naloxone) kits
- Medications for Opioid Use Disorder Program
