The Harnett County EMS Community Paramedic Program plays a vital role in bridging gaps in healthcare by providing preventative, urgent, and follow-up care directly to members of our community. Unlike traditional paramedics on ambulances who primarily respond to emergencies, community paramedics focus on proactive healthcare, ensuring that individuals - especially those in rural or underserved areas - receive the care they need before a medical crisis occurs that requires a 911 call and emergency response. 

 

 What Does a Community Paramedic Do?

  • Home Visits & Preventative CareConduct wellness checks, monitor chronic conditions, and help patients manage medications.

 

  • Reducing Emergency Room Visits - These home visits help reduce and prevent the unnecessary emergency room visits. 

 

  • Connecting Patients to Resources - Community Paramedics assist with referrals to primary care doctors, mental health services and social support programs. 

 

  • Health Education & Support - Community Paramedics educate patients and caregivers on managing chronic diseases, proper nutrition, and medication adherence. 

 

  • Follow-Ups - Community Paramedics check in on patients at home to ensure continuity of care.
